Apply for a Guide Dog Now!
This application is for people who are blind or visually impaired, and want to apply for a Guide Dog from our program.
Instructions:Use the form below to submit your application. This process takes 30-40 minutes to complete. Once you have started the form, there is a link on the bottom to "save draft" if you need to resume later.
Steps to save and continue later:
1: Click or tap the save draft button on the bottom left of the application
2: This will take you to a confirmation page with a link to resume the application. You can either copy that link or locate the email field, which will allow you to confirm where the draft resume link should be sent.
3: Verify the correct email address is displayed, or provide one, and press the "send" button. You will receive an email from Guide Dogs of the Desert with the link.
